About the role

Dementia Forward is seeking a proactive and enthusiastic person to join their team as a Memory Support Advisor. The role involves working across primary care settings, liaising with staff to streamline diagnosis and providing support and advice for people who are waiting for a memory assessment. The role will involve holding clinics and offering home visits and will also include work on dementia prevention. They’re looking for someone who is empathetic, supportive and an excellent communicator. The successful candidate will also be I.T. literate and willing and ready to learn new systems.
