Are you working as a social prescribing link worker, social prescriber, or in a similar community-based role in Humber and North Yorkshire?

The Humber and North Yorkshire Integrated Care Board (ICB) is updating its Social Prescribing Link Worker Register and would be grateful if colleagues could complete a short annual update form.

They are keen to capture the broadest picture of the social prescribing workforce across Humber and North Yorkshire, including colleagues working within PCNs, VCSE organisations, local authorities, NHS services and other community-based settings.

Maintaining an accurate register supports system communication, neighbourhood integration, workforce planning and future developments across the region, including delivery of the NHS 10 Year Health Plan.

Please share with social prescribing link workers, social prescribers and similar community-based roles within your organisation or network.

The form takes approximately three minutes to complete.

The deadline for responses is 30 July 2026.
